Bactin

Indications

Bactin and trimethoprim act on two different stages in the synthesis of bacterial folate, preventing the bacteria from producing folate. Without folate, the bacteria cannot produce DNA and so are unable to increase in numbers. Co-trimoxazole therefore stops the spread of infection. The remaining bacteria are killed by the immune system or eventually die.
Bactin infusion, tablets, forte tablets and suspensions all contain the active ingredients sulfamethoxazole and trimethoprim, which together are known as co-trimoxazole. Co-trimoxazole is available without a brand name, ie as the generic drug. Sulfamethoxazole and trimethoprim are both antibiotics that are indicated to treat infections caused by bacteria.
Bactin is a type of antibiotic called a sulphonamide. Bactin is related to the sulphonamides. These two antibiotics work in synergy together against certain types of bacteria.
Bactin is indicated to treat a small number of serious infections, but serious side effects limit its use. Less serious infections such as urinary tract infections and otitis media are only treated with co-trimoxazole when there is good evidence from microscopy and culture that the bacteria are sensitive to co-trimoxazole, and there is good reason to prefer the combination to a single antibiotic.
Bactin, an antibacterial combination drug, is indicated for the treatment of certain urinary tract infections, severe middle ear infections in children, long-lasting or frequently recurring bronchitis in adults that has increased in seriousness, inflammation of the intestine due to a severe bacterial infection, and travelers' diarrhea in adults. Bactin is indicated for the treatment of Pneumocystis carinii pneumonia, and for prevention of this type of pneumonia in people with weakened immune systems.
In order to grow and multiply in numbers, bacterial cells need to produce genetic material. To produce DNA they require folic acid. However, bacterial cells can't take up folic acid supplied in the diet like human cells can. Instead, they synthesise it themselves.
To make sure the bacteria causing an infection are susceptible to co-trimoxazole, your doctor may take a tissue sample, for example a swab from the infected area, or a urine or blood sample.

Bactin is indicated for the treatment of:
- Flare-ups of chronic bronchitis;
- Infection of the bladder or tubes that pass urine;
- Middle ear infections;
- Prevention and treatment of pneumonia caused by the bacterium pneumocystis carinii. this infection is most common in people with decreased immune system function, for example due to hiv or aids;
- Prevention and treatment of toxoplasmosis;
- Treatment of a lung infection called nocardiosis;

Side Effects

Bactin side effects that you should report to your health care professional or doctor as soon as possible:
- Cough;
- Decreased amount of sodium in the blood;
- Depression;
- Diarrhoea;
- Disturbances in the normal numbers of blood cells in the blood;
- Dizziness;
- Hallucinations;
- Headache;
- Hives;
- Increased amount of potassium in the blood;
- Inflammation of the lining of the mouth;
- Lack or loss of appetite;
- Liver or kidney disorders;
- Loss of appetite;
- Low blood sugar level;
- Nausea and vomiting;
- Nausea;
- Overgrowth of the yeast candida, which may cause infection such as thrush;
- Pain in the muscles;
- Rash;
- Seizures;
- Severe blistering skin reactions, eg stevens-johnson syndrome, toxic epidermal necrolysis;
- Shortness of breath;
- Skin rash;
- Vomiting;

Dosage

ADULTS

Travelers' Diarrhea

The usual recommended dosage is 1 Bactin DS (double strength tablet), 2 Bactin tablets, or 4 teaspoonfuls (20 ml) of Bactin Pediatric Suspension every 12 hours for 5 days.

Prevention of Pneumocystis Carinii Pneumonia

The recommended dosage is 1 Bactin DS (double strength tablet) once daily.

Treatment of Pneumocystis Carinii Pneumonia

The recommended dosage is 15 to 20 mg of trimethoprim and 75 to 100 mg of sulfamethoxazole per 2.2 pounds of body weight per 24 hours divided into equal doses every 6 hours for 14 to 21 days.

Worsening of Chronic Bronchitis

The usual recommended dosage is 1 Bactin DS (double strength tablet), 2 Bactin tablets, or 4 teaspoonfuls (20 ml) of Bactin Pediatric Suspension every 12 hours for 14 days.

Urinary Tract Infections and Intestinal Inflammation

The usual adult dosage in the treatment of urinary tract infection is 1 Bactin DS (double strength tablet) or 2 Bactin tablets, or 4 teaspoonfuls (20 ml) of Bactin Pediatric Suspension every 12 hours for 10 to 14 days.
The dosage for inflammation of the intestine is the same but is taken for 5 days.

OLDER ADULTS

Consult with your doctor before taking Bactin.
There may be an increased risk of severe side effects when Bactin is taken by older people, especially in those who have impaired kidney and/or liver function or who are taking other medication.

CHILDREN

Intestinal Inflammation

The recommended dosage is identical to the dosage recommended for urinary tract and middle ear infections; however, it should be taken for 5 days.

Urinary Tract Infections or Middle Ear Infections

The recommended dosage for children 2 months of age or older, given every 12 hours for 10 days, is determined by weight.

The following table is a guideline for this dosage:
88 pounds, 4 teaspoonfuls (20 ml) or 2 tablets or 1 DS tablet
66 pounds, 3 teaspoonfuls (15 ml) or one-and-a-half tablets
44 pounds, 2 teaspoonfuls (10 ml) or 1 tablet
22 pounds, 1 teaspoonful (5 ml)

Prevention of Pneumocystis Carinii Pneumonia

The total daily dose should not exceed 320 mg trimethoprim and 1600 mg sulfamethoxazole.
The recommended dosage, taken twice a day, on 3 consecutive days per week, is determined by body surface area.
The safety of repeated use of Bactin in children under 2 years of age has not been established.

Treatment of Pneumocystis Carinii Pneumonia

The recommended dosage, taken every 6 hours for 14 to 21 days, is determined by weight.
